Why anti-virus software and regular patching don't work
Anti-virus software affects the performance of the system (it becomes slower, poorer availability, ...) and signatures have to be updated at least once a day. Patches have to be reinstalled at least every 4-8 weeks and they cannot be used on certain systems (no more patches are available, loss of warranty, …).
Why firewalls aren't the only solution
It is necessary to open the requisite ports in the firewalls. Malware uses the same ports for its dissemination (e.g. TCP/445). Firewalls generally limit the availability. It is therefore necessary for them to be configured on a redundant basis (higher complexity, but lower availability). As a result of their regular updating, firewalls become more “open” over time because rules are entered but rarely removed).
